Upper Coomera Respiratory Clinic

For the Upper Coomera Respiratory Clinic patients – both forms need to be completed:

1. Please navigate to: Upper Coomera Respiratory Clinic form – so you can have a Bulk-Billed Covid-19 Test (otherwise there will be a significant charge) and a Prescription (if required)
